A nice walk in the woods into the lower Kinni under partly sunny skies made me cry realize how out of shape I would be on the walk back up the hill. Time to shed the winter weight. Oddly enough I had just read a diary entry from May 2. Abbie Leavitt about the exact spot we were headed to. On to lip rippin. For the first hour, the 3 of us fished long, medium deep runs that were mostly sandy and brushy with a few rocks and stumps for current breaks. In the spring and summer the trout locate here in good numbers. On Friday, no fish were seen, spooked or caught out of this water. The water enters from a fast, flat, rocky run about 5. The stream bank of the bluff has numerous old growth willows, sprouting from between the shoreline rocks. A trickling spring emerges at the elbow of the bend. The spring is covered in brush from floods past and old willow trunks. Few know the spring is there and only patient and observant listeners will hear the gurgling trickle from the base of the bluff. On the upstream side of the elbow there is a 4. This fluff and food circulates round and round and round. The majority of the back eddy is 1- 2ft deep, but where the seam shows on the surface current, it abruptly drops off to the depth of the pool. Delaware River, East Branch, New York Fly Fishing Reports & Conditions.This can be a productive seam. This run into pool scenario will be used to attempt to answer a question of approach in my last ask andy post. As we walked down to the pool we observed changes in the river as compared to last year. The water seemed deeper and faster in the run leading up to the bend pool, more rocks and scoops. A good sign. Last year by mid summer this bend pool had almost filled in with sand and was pointless to fish by the end of the season. In the heat of the summer this might be good, but in the winter, the sun might give us and the fish some much needed warmth to be active. A bad sign. When we arrived at the complex we observed the run and pool as a whole, from the high bank on the inside of the bend, and came up with an approach based on our observations. This is the approach I would have used if fishing alone with a nymph. My position is on the inside of the bend. I would split the river up into 3 sections from close to far across the river, the close side, the middle, the far side. I would start at the very back of the pool and fish the tail out first,,,,on the close side. I would make depth adjustments and proceed to the middle and far side from approximately the same position. Depending upon my success at catching I would move my feet up stream a step or two and adjust my drifts and mends. Next, I would move my feet up, make adjustments for indicator depth and weight and fish the depth of the pool,,,,,,,,,, close, middle, and far. If I was not catching, I would keep making adjustments which also could include changing fly patterns and focus on the head of the poolor the drop zone,,,,,,,close, middle, far. I believe the angler will have more success at catching if he/she has a focused approach on specific features of a run pool complex. Fish those features well, then move on. To try and fish all of the features in a run pool complex requires different adjustments for different features. Fishing slow water requires little or no weight. Fishing the depth of the pool requires more length between the fly and the indicator. To try to drift long sections of highly variable and dynamic water, with one setting, will leave the angler fishing most of the water with the fly outside the zone where the trout most likely will be. Similarly, standing in one spot, with the same set up, throwing the same cast, to the same place, over and over again, will cause the angler to miss many of the fish holding features and opportunities of a complex like this. Making adjustments to your feet, your weight, indicator length, mending, fly/s etc. My drifts become longer in duration because I am now casting to the head and swinging through the tail,,,,,inside, middle and far. Previously I would just be dead drifting the fly a short distance. Now I am dead drifting and imparting action to the fly as it moves through different zones. Next I concentrate on the drop zone. At this point I will be standing on the close side, back from the edge of the drop of the depth change. I will work this area most diligently. My mending will become more aggressive. Work from close to far, inside seam, middle, outside seam and then into the slow swirl of the back eddy. Don’t forget the back eddy!! Now I am at the run above the pool. In this case it is flat rocky and two feet in depth. From close to far it is generally featureless to my eye from above but I still keep my same method of approach. My experience tells me it is too fast moving of water for these temp and this time of year. I fish it anyway because you never know and it is part of the approach . I make my adjustments to keep the fly just off the bottom, fish from close to far, move my feet, Change patterns if no eats. SIMPLE. When I get to the top of the run, I reverse order and do the same exact focused approach from upstream to down. I probably will have more line on the water and impart more action to the fly. I will mend more to let the fly sink and will probably make fewer adjustments to my weight and indicator as I come back down stream. Some days I will start at the top and work down. Remember when you work from top to bottom, the fly is the first object into the trouts window of vision. Working from bottom to top, not only do you have to cast line or leader over the fish but it is likely your line is more visible than the opposite presentation. On this day there were 3 anglers. We took turns in the big run and pool for nearly 3 hours. We mostly covered the big pool because we all thought the fish would be there. Cast and mend, inside to outside. Standard rigs were bead heads of different sorts, p. We covered the water well. My friend Randy had touched two to three on the inside seam in the slow water just below the drop zone. The bites were super soft. None came to hand. I asked him what pattern he was fishing. He had just switched to a non bead #1. Inside seams 1. 0 feet from my wading boots are rarely the most productive fish catching spots for me. They however, are part of an approach that enables this angler to cover the water in the most effective manner for catching. Many days, in this exact complex of run into pool, the fish are not caught on the inside seam. As to the whys, I cannot be certain. I am left to ponder, knowing I will never know definitively. It is part of the fun. Was it the fly, the time of day, the drift, the tidal pull??? The thing I do know is that I sticking with a broad and general approach coupled with diligent focus eventually pays off like it has many times before. There are four general types of injury, and an individual brachial plexopathy may include any or all of these injury types, on one or both (bilateral) sides of the body. The types are as follows: A stretch, praxis (damage), or traction (pulling or tension- creating) injury, in which the nerve has been overstretched and damaged but is not torn. Most brachial plexus injuries in newborns are of this type. A rupture, in which there has been a separation within a nerve. A single nerve may have more than one rupture. A neuroma, in which scar tissue has formed around a nerve injury. The scar tissue puts pressure on the nerve and interferes with nerve signal conduction to the muscles. An avulsion is the most severe form of brachial plexus injury. It is the detachment of a nerve from the spinal cord. A minor brachial plexus injury can be a stretched nerve that results in a short- circuit in a few of the nerve fibers, causing temporary paralysis. A more severe injury may involve a complete disruption of the nerve, in which the nerve fibers around the injury become disorganized, all nerve function is lost, and scarring prevents the nerve from healing. Such an injury can result in permanent paralysis. Obstetrical brachial plexopathy. Most injuries to the brachial plexus during birth involve the C5 and C6 nerve roots, affecting the movement of the shoulder, upper arm, and elbow. Limited shoulder motion can affect the function and strength of the hand. Obstetrical brachial plexopathies are classified according to the extent of the injury. ERB'S PALSY Obstetric Erb's palsy is an injury in the upper brachial plexus involving C5 and C6 and sometimes C7. It usually affects the upper arm and the rotation of the lower arm. Erb's palsy is known also by the following names: Erb- Duchenne palsy or paralysis. Duchenne- Erb syndrome or paralysisupper brachial plexus palsy or paralysis, Erb- Duchenne type. KLUMPKE'S PALSY Klumpke's palsy or Klumpke's paralysis is an injury to the lower brachial plexus: C7, C8, and sometimes T1. Children with Klumpke's palsy have normal use of the shoulder and elbow but weakness or paralysis in the hand and fingers. An infant also may have a drooping eyelid on the opposite side of the body from the affected hand. TOTAL PLEXUS PALSY Also called Erb/Klumpke palsy, total plexus palsy involves all of the nerve roots of the brachial plexus to at least some extent. The entire upper extremity is affected. It is estimated that in North America between one and three of every 1,0. Although obstetric brachial plexopathy usually heals quite rapidly on its own, the infant may begin physical therapy within the first two weeks of life and should be evaluated by a specialist, such as a pediatric neurologist, by six weeks of age. A newborn lacking movement in an arm is examined first for fractures of the collarbone, ribs, humerus (the long bone of the upper arm, extending from the shoulder to the elbow), and even the femur (thighbone), as well as dislocations of the shoulder or elbow. Symptoms of such fractures and dislocations may be similar to those of brachial plexopathy and can cause infants to not move their arms. Those at risk for obstetric brachial plexopathy are also at risk for fractures and dislocations during birth. ![]() If you are a German Shepherd owner or someone considering acquiring a shepherd, you may have heard and be concerned about the propensity of the breed for hip dysplasia. Although this is a common problem in the GSD, its impact upon the quality of the dog. I write this article both as the director of a non- profit devoted to saving GSDs and educating people about the breed and as the owner of a beloved shepherd named Katie who suffers from DM. No one is exactly sure what causes Degenerative Myelopathy (also referred to as CDRM . The most significant research on this disease is being done by a veterinarian at the University of Florida . His theory is that DM is an autoimmune disease that occurs when the dog. Many have compared it to the symptoms and deterioration that occur in humans who suffer from multiple sclerosis, although others in the vet community contend that DM and MS are really two very different diseases and some, though they really have no explanation for what causes DM, challenge Dr. Obviously, there is no consensus of opinion about exactly what DM is or what causes it. The subject on which most everyone can agree, however, is that DM is a neurological disease which presents with a set of symptoms that are easily recognized by vets who see a lot of dogs who suffer from DM and often misdiagnosed by vets who are not familiar with DM. The disease can develop in almost any age dog, but seems most common in dogs 6 years and older. Initially, signs may be similar to symptoms associated with hip dysplasia and or arthritis (hence, the frequent misdiagnosis) . One of the symptoms which may clearly signal a DM problem is that the dog begins initially to drag its rear feet in such a way that the two center nails on both rear feet scrape and are often worn down to the point of bleeding. In time, the dog. Some dogs begin to walk with a . The dog may also turn its rear feet (. A vet who suspects DM (or other neurological problems) may test for the neurological symptoms of DM by turning/ folding a dog. Essentially, DM is a diagnosis of exclusion . Similar neurological symptoms can be caused by a disc or nerve compression, by certain spinal deterioration, by a tumor, etc. The only way to determine whether one of these other problems is causing the symptoms is to have an MRI and/or a myelogram performed on the dog. Both of these tests are expensive (probably ranging from $1,0. DM, even if a tumor or disc/nerve problem is ruled out. It simply rules out other likely causes of the same symptoms. The other difficulty, besides cost, is subjecting your dog to either of these tests, is that it causes a stress upon the animal and stress, above all else, is thought to be one of the biggest aggravators of DM, frequently causing an irreversible worsening of the symptoms. Therefore, it becomes a tough decision whether to have the tests performed or not. Additionally, the surgery to correct the problem is another stress which can aggravate the DM if it is also present. Once the conclusion is that the dog probably is suffering from DM (based either on ruling out other factors or simply by virtue of the symptoms the dog is demonstrating), treatment becomes the next question. There is no cure for DM . Some dogs will lose control of bladder and bowels, others will not. In some dogs, deterioration from the scraping feet and a slightly wobbly backend to complete paralysis will happen in months, in some dogs it may occur over a one to two year period. The neurologist we saw with our girl Katie, who sees about 3 dogs EVERY month suffering from degenerative myelopathy, said the very longest he ever saw a dog last from initial symptoms until the dog was put down was three years, but he stressed repeatedly that this was the rare exception and that most dogs deteriorate rather quickly, usually in less than a year from initial onset of any symptoms. As the symptoms progress, the dog may become completely paralyzed in the rear quarters and that may spread, eventually, to the front limbs as well. Additionally, many dogs lose control of bowel and bladder functions and some begin to develop bedsores and infections from lack of mobility. Since there is no cure, the question becomes whether there is anything you can do to slow the progression of this dreadful disease. The answer to that depends upon who you ask. Conventional wisdom in much of the traditionally trained vet community is that there is nothing you can do other than trying to minimize your dog. Clemmons, on the other hand, believes strongly that the progression of the disease can be slowed by a set of approaches designed to treat autoimmune problems generally. This approach really involves a combination of prescription medications (acetylsysteine and aminocaproic acid (known by the tradename . One of the exercises that is most strongly advocated for dogs with DM is swimming as it allows for the dog to maintain muscle tone while exercising in an manner which is unlikely to cause any injury. People whose dogs suffer from DM have reported varying degrees of success with this combination of approaches and some people choose to use one or two of the approaches only. The vets that we have spoken with are not convinced that any of this really makes any difference but nobody really knows so most people with DM dogs are inclined to try it in hopes of extending their dog. Clemmons emphasizes that the likelihood of his treatment plan helping to lessen and slow the symptoms of DM is far more effective if begun very early on when the dog first shows symptoms, rather than after the dog has really begun to show severe symptoms and signs of paralysis. Turning to our own personal story, our Katie Kakes began showing signs of problems with her rear legs almost a year ago. At the time, she was suffering from impacted anal glands and we thought that part of her . We repeatedly had the anal glands expressed and, ultimately flushed. Nonetheless, we continued to see a worsening in her mobility with her rear legs . We took her to two different vets, both of whom attributed her symptoms to a combination of arthritis and/or hip problems (though Katie has never had a hip problem at all in the 6 years we had her, having acquired her at 2 years of age when she was dumped in a shelter in York, Pennsylvania). All of these problems began around March/April of 2. By July, we were noticing more problems with her legs . Neither of these was an all the time occurrence but it disturbed us enough to go back to the vet and essentially demand that it be recognized as something neurological. After doing some basic neurological testing, the vet did agree that this wasn. However, she indicated it could be age related spinal changes and suggested a full spine x- ray. And so it was that Katie began a series of tests and procedures. The spinal x- ray showed a very healthy spine, especially for an 8 year old dog. Therefore, that didn. In the meantime, the anal gland problem was also worsening and beginning to show signs of being a perianal fistula (a medical condition common in many dogs and not easily treated). The vet suggested that the combination of the neurological problems and the perianal fistula warranted a consult with a surgeon and so it was off to the specialist. Meanwhile, on the weekend before the consult appointment, Katie had to be rushed to the ER because one of her anal sacs had burst and she was bleeding rather profusely from the anal area. That problem was resolved fairly quickly and we were able to take her right back home again. A few days later, the surgeon who saw Katie agreed that she suffered from perianal fistulas and that surgery would be the only way to really correct that. However, he felt that her neurological problems required more immediate diagnosis and attention. He agreed that her symptoms could likely be DM, but he indicated that they were also consistent with a disc problem and so he recommended getting an MRI (a myelogram . So, off Katie went to a facility which specializes in doing MRI. The results of the MRI showed that she had a disc related nerve root compression at the L7 area. Although this wasn. We suddenly began to think that our dog. He suggested that Katie. He cautioned that it was remotely possible that there could turn out to be signs of cancer at the site of the compression, but that wasn. And thus it was that we came to schedule Katie for surgery on Tuesday, September 1. We had no idea, when we dropped Katie off at 8: 3. Although no biopsy could be done because the material had hardened too much, the vet assured us that he saw enough malignancy to recognize it pretty much on site and he assured us that Katie did not have cancer. We were reassured and hoping that September 1. Katie, though it had been such a terrible day overall. Two days later we were able to pick Katie up and bring her home. She could walk with the help of a sling. She was forbidden from climbing stairs (Bob carried her up and down the stairs each day . During that time, she was able to walk and seemed about the same as she had pre- surgery with respect to the control she had over her hind legs, but we thought she would continually improve as she recovered from the surgery. On her return visit in two weeks, Katie walked much the same as she had pre- surgery. The surgeon suggested that it was too early to tell whether she really did suffer from DM or whether she still had swelling from the surgery that affected her mobility. Her suggested that we get her on a regular regimen of swimming to start building her muscle tone and to speed her recovery.
